Report a Claim:

Please report all claims within 24 hours of the employer receiving notification of injury.

ONLINE: Use our secure web portal located in the Client Services Area.

FAX: Complete the First Report of Injury Form (use the download link at the right) and fax us at 877-884-6573.

CALL CENTER: Call us at 877-585-1117 and provide the claim information to a Claims Services Representative over the phone.

E-MAIL: Completed First Report of Injury forms can be e-mailed to RAS at: firstreports@rascompanies.com

  • Download and print State Specific First Reports of Injury Forms from the State Website. Once you have printed the form you can fill in the information and forward to RAS. These forms can not be completed online. To file first reports of injury online go the the Client Services tab and log in to the RAS client portal.

  • Report Fraud

  • State Statutes require that an employer must file a First Report of Injury whenever an employee claims to have been injured at work.

  • Whether or not the employer disputes the claim has no bearing on the employer's obligation to file the First Report of Injury with its insurer.

  • Filing the First Report of Injury does not admit liability.

  • Failure to file the First Report of Injury or late filing of the First Report of Injury may subject the employer to penalties and/or lawsuits under the workers’ compensation law.

As your workers’ compensation administrator, we will investigate all claims. If you have concerns about a claim, please advise us of those concerns, and we will investigate the claim thoroughly.
